[Devel] assigning MIDI CCs to lv2:ControlPort

There is already an extension for this:
http://ll-plugins.nongnu.org/lv2/ext/midimap/

I'm using it in my (experimental) host, where all parameters are automable
via midi CC.
The plugins that provide such information (currently only DSSI ones and
sineshaper-LV2) will have pre-set midi CC automation ready.
